I am looking for a custom made shock to suit my 1985 kdx200. Ideally with an internal bladder so that a remote reservoir is not required. Something like a hagon or F1 style made for the klr650.
Who makes custom shock ?

As a general rule, any of the aftermarket shock manufacturers will probably be willing to help you out for a price...
But you should send Walter ("Wasp") a PM. He's Mr YSS/Hyper Something/TechnoFlex in Australia.

Works Performance in the USA are also good, but a lot harder to deal with.
There's also a few places in the UK, but they tend to be very expensive.
Why no resevoir? I don't think the KDX shock is long enough to get by without an external resevoir. The A models had their resevoir where the battery lives on a B model - if you're going without a battery, it wouldn't be hard to move the resevoir back to the A's spot (and away from the carby).

Today's discovery: KDX200C/D shock fits and has both comp and rebound adjustment (and the rebound adjustment is way better than the A/B shock). Will lift the rear of the bike by about 10mm
Second discovery is that an early Full Floater RM shock can be made to work. 2-way adjustable, 16mm shaft and 46mm body. Lifts the rear of the bike by about 20mm which suits me perfectly - it will match the 20mm rise from fitting the '84 KX disc brake front end.
Just some options that might get you mobile for less money.

Definetly give Walter a PM/call. I went and saw Walter (with the rear shock from my 1983 XT600 Tenere as a sample) on Wednsday morning and returned at 3pm to pick up a nice shiny new Promax shock that Walter had built to suit my weight etc. Great guy, great service.

Don't do this to yourself... For the love of god, don't do this to yourself.
I've just finished the conversion, and it was a total turd of a job... Ended up with KDX-C spring, modified RM spring seat, custom spacers and a lot of swearing.
I put it in on the expectation that it will need revalving, but now dearly hope that it won't, 'cause its a mongrel to get in and out of the bike.
